Requisites

Study Guide and obligatory requisites for your inclusion in a program of assisted reproduction in order to provide you with the best attention accordin to your particular case.

Medical requisites
You will need to provide a copy of every exam to your physician and/or public relations coordinator. The exams are valid for six months only and are NOT included in the treatment cost.

Patient
Basal Hormonal Profile (taken during day 3-5 of the cycle)
HIV Test
Hepatitis B & C Test

Administrative Requisites
Copies of:
Marriage Certificate
Picture ID (Spouses)
ID Form (ckick on the link to get the form)
Psychologic Profile
Letters of consent properly filled by each of the spouses.

Treatment will be schedule once all requisites are appropiately met.
